]]>CONSENT TO FLOATATION SERVICE, RELEASE, AND WAIVER
Floatation Therapy is the relaxing practice, while being buoyant in approximately 10 inches of water in a solution of Epsom Salt. Floats are typically around 60 minutes. The temperature of the water is set to the normal body temperature. This is accomplished by our floating room.
I agree to the following:
- I will take extra precaution when in the float room. Floors may be wet at times due to entering and leaving float. I assume all liability due to injury and/or damage resulting in any slip and fall.
- I am physically able to get in and out of float tank on my own.
- I agree to shower with soap thoroughly before entering each float session to completely remove dirt and oil from body.
- I agree to remove all oils, creams, and makeup from body before floating.
- If pregnant, I will provide Locs of Glory with a consent form from my doctor.
- I agree that I do not have any spreadable diseases, skin disorders, or illnesses.
- I am aware that newly treated hair color is not suggested for floating. Hair color needs to be at least 4 weeks old before floating.
- I agree to make establishment aware of all medical issues and provide medical consent.
- If pregnant, I will provide Locs of Glory with a consent form from my doctor.

I do not have an allergy to Epsom Salt.
- I agree to remove all oils, creams, and makeup from body before floating.
- I agree to make establishment aware of all medical issues and provide medical consent.
- I agree that I do not have any spreadable diseases, skin disorders, or illnesses.
- I understand floating may lower my blood pressure.
- I acknowledge that I’m not under the influence of drugs and alcohol.
- I am aware that newly treated hair color is not suggested for floating. Hair color needs to be at least 4 weeks old before floating.
